Heart Evangelista becomes unrecognizable as the simian-looking Carissa in the ABS-CBN teleserye Hiram na Mukha. It is the second offering of Sineserye, the TV serialization project of the Kapamilya network. Sineserye Presents: Hiram na Mukha is based on the 1992 movie by Joel Lamangan of the same title.

TJ Trinidad (leftmost) stars in Hiram na Mukha as Dr. Hugo Roldan, a wealthy and renowned plastic surgeon who changes Carissa's ugly looks into the beautiful face of his deceased wife, Alicia. Carissa (Heart) is caught up in a love triangle between Dr. Hugo Roldan and Mendez (Geoff Eigenmann), Carissa's neighbor and close friend.

Geoff and Heart are reunited again in Hiram na Mukha. They first starred in the now defunct teleserye Hiram. Meanwhile, TJ Trinidad again plays a doctor, similar to his role in another ABS-CBN teleserye Walang Kapalit.

Cherry Pie Picache (leftmost) and Katya Santos are the sisters Rose and Morita Ponce. Rose is the caretaker of Hugo's vast wealth while Morita is madly in love with Hugo (TJ Trinidad).

Cherry Pie Picache (leftmost) was part of the original cast of Hiram na Mukha, which was shot 15 years ago. She recalls that during that time, she was just a bit player and that her scenes were shot in the slums. In the modern version, she now plays a major character, Rose Ponce, and the scenes were shot in posh sets. Katya Santos, on the other hand, is Rose's younger sister Morita.

Heart Evangelista recalls the difficulty of shooting Hiram na Mukha: "Actually, yung umpisa nga ng Hiram na Mukha, hirap na hirap ako umiyak. Sa Hiram dati, walang motivation umiiyak na ako agad, e. Ngayon, sobrang hirap akong umiyak....although ngayon bumalik na. I guess there was a time na nagpipigil akong umiyak. Naging bato na ako or something. It was very hard na makaiyak ulit ako kasi hindi na ako naaapektuhan sa maliliit na bagay."

Carissa (Heart Evangelista) accepts Hugo's offer to be his test patient for a new cosmetic surgery procedure he has developed. Carissa's monkey-like face is now transformed into Alicia, Hugo's deceased wife. Will Carissa find true happiness now that she is beautiful?

Dr. Hugo Roldan (TJ Trinidad) is a renowned plastic and cosmetic surgeon. He is developing a new surgical procedure that he claims is revolutionary but he still does not have a test patient. When he sees the simian-looking Carissa after being involved in a car accident, he offers to try the procedure on her. When Carissa agreed to his proposal, he patterns her face on his dead wife, Alicia.

Mendez (Geoff Eigenmann) is Carissa's neighbor who is the only person to see the true beauty behind Carissa's "ugliness." What will be his reaction to Carissa's changes after Hugo "fixes" her monkey-like features?
