Layunin daw ng music video na i-highlight ang kabayanihan ng frontliners, ang bayanihan spirit, at ang pagiging positibo sa buhay ng mga Pilipino.
Sa panayam kay DOT Assistant Secretary Howard Lance Uyking ng 24 Oras reporter na si Aubrey Carampel:
"We wanted to pay tribute to their sacrifices that's why we came up with a video just to show random acts of kindness.
"And napili namin yung 'With A Smile' because it encapsulates what Filipinos really are.
"Na we're resilient kahit anong challenges. We approach everything pa rin with a smile."
Ang umawit Eraserheads hit song sa music video ay ang singer-songwriter na si Bea Lorenzo, na ang ama't lolo ay parehong doktor.
